About Cornerstone Care Clinic

  • How is Cornerstone Care Clinic different?

    At Cornerstone Care Clinic, we seek to provide high-quality primary care services to members of our community who are uninsured or underinsured. As a Direct Primary Care clinic, we do not bill insurance. Instead, we provide low-cost monthly memberships and cash pricing for our healthcare services, allowing our practitioners to spend their time with you, the patient, instead of your insurance company.

  • Who provides care in the clinic?

    All of our providers are board certified Family Nurse Practitioners from the American Academy of Nurse Practitioners. Michelle Morris is our Clinic Director and nurse practitioner with over 10 years of experience in primary and urgent care.

  • Where are you located?

    Cornerstone Care Clinic is located at 1520 Ohio Avenue South in Live Oak, Florida.

  • What are your hours of operation?

    Cornerstone Care Clinic is open Monday - Friday from 9:00am to 5:00pm. 

  • What forms of payment do you accept?

    We accept cash, check, all credit cards, HSA, FSA and ACH transfer. 

  • Can I use my HSA or FSA card to pay for services?

    Yes. You can use your HSA or FSA to pay for individual services or procedures at our clinic. However, membership fees cannot be paid with an HSA or FSA.

  • What is Direct Primary Care?

    Direct Primary Care is a model of care where a patient engages a primary care provider directly. This can be in the form of transparent cash pricing or a low-cost monthly membership. Memberships offer advantages such as unlimited visits at no additional cost as well as discounted medication and labs.

  • What procedures or exams do you provide on-site?

  • Do you offer walk-in appointments?

    Yes. We do our best to provide same-day or next-day appointments for walk-in patients with an immediate care need.

  • Do you offer telehealth appointments?

    Yes. We have invested in technology that allows us to provide telehealth visits for your convenience.


    Please note that there may be certain conditions or cases where telehealth is not an option and you will need to be seen in person, but we will do our best to deliver your care in the most convenient way possible.

  • How quickly will I be seen?

    We do our best to offer same-day or next-day appointments.

  • What if I need to see a specialist?

    A critical part of good primary care is knowing when a particular condition is out of our scope of practice and requires expert care from specialists. Our Clinic Director has established a network of specialists who we recommend and will strive to make the transition of care as easy as possible.  As a benefit of clinic membership, we will also advocate for you to receive low-cost cash pricing from these specialists.

  • What age range of patients do you accept?

    We currently provide care to patients 5 years of age and above.  We hope to provide care for all ages in the near future.

  • How do I schedule an appointment?

    To schedule an appointment, please call 386-219-4060.  


    If you have already visited the clinic, or are a member, you can use our online portal, Passport, to access your patient records and schedule appointments. For help with Passport access, please Contact Us.

Membership & Insurance

  • Can I be seen in the clinic without a membership?

    Yes. We offer low-cost monthly memberships, but we also have cash pricing for each of our healthcare services.

  • How do I sign up for a membership?

    Sign up is easy. Please click on the  "Become a Member Today" button at the top of any page to fill out the online form. Once you complete this form, you will have access to all the benefits of membership. 

  • How much does a membership cost?

  • What services are included in my membership?

    Our cost-effective membership plans provide you with unlimited visits and access to almost all services at no additional cost, plus added benefits.

  • Is there a minimum term length for membership?

    We believe faithful membership should be based on the value we bring to you, not hidden contract terms that lock you in for years. We ask members for a 3-month commitment to fully assess the value of your plan. After that, you can cancel your membership at any time. 

  • Are there any other fees for members?

    No. At Cornerstone Care Clinic, we believe payments should be simple without any hidden fees.  The only fee you will pay is your monthly membership fee. 


    Most services are included at no additional cost in the membership, but there may be some things like medications, labs, or tests that you may pay for at deeply discounted rates.

  • Is my membership at Cornerstone Care Clinic considered health insurance?

    No. Your membership with Cornerstone Care Clinic should never be confused with health insurance. A membership at Cornerstone Care Clinic will provide you with access to primary and immediate care, as well as discounted labs and medications, but it is not health insurance. 


    It is still important to protect yourself financially from expensive, catastrophic medical events like hospitalizations, surgeries, ER visits, or cancer treatments.   • Do I need insurance if I have a membership with the clinic?

    Yes. A membership at Cornerstone Care Clinic will provide you with access to primary and immediate care, as well as discounted labs and medications, but it is not health insurance. 


    It is still important to protect yourself financially from expensive, catastrophic medical events like hospitalizations, surgeries, ER visits, or cancer treatments.   • Can you bill my insurance?

    No.  We are not able to bill insurance for any of the services we provide. 


    In traditional healthcare models, providers must spend a majority of their time documenting appointments for the sole purpose of billing insurance. Cornerstone Care Clinic removes the constraints of insurance and instead provides maximized, personalized care through low-cost monthly memberships and transparent, cash pricing.

  • How do I cancel my membership?

    We hate to see you go, but we understand that the Direct Primary Care model may not be a good fit for everyone.
